Pre-访问形式

我们鼓励您在访问前填写所有适用的访问前表格 学生健康服务. 为了完成预访问表格，你需要点击下面的链接，创建一个唯一的用户名和密码，如果你还没有这样做，并完成适用的 pre-visit形式. 

The Health 历史 pre-visit形式 may also be found underneath the pre-visit形式 section; however, 每学年只应完成一次.  也, the Authorization for the Release of Protected Health form (Authorization to Disclose Information form) maybe found in the same section; but it should only be completed when you are requesting records to be released to/from 学生健康服务 to/from another party (i.e. 医疗保健提供者、自己、父母、学校等.).
